Age UK Bexley requires a Trusted Assessor to enable people referred to us by Bexley Adult Social Care, to find the right support services that meet their needs. You will collaborate with colleagues, and assist with development and review of support plans.
You will demonstrate a commitment to working with older people, an understanding of support planning, experience of working with people from different backgrounds, and have excellent IT skills including using a case management system for recording purposes.
